Process Requirements
FAL’s aim is to provide a high level of service to our Clients. We want to maintain a dedicated team of professionals specialising in the role of surgical assistants. With this in mind FAL has in place strict protocols to ensure that all our candidates are properly assessed, checked and identified before they attend any of our Client bookings. Following successful registration, candidates will be required to complete a detailed application form, a detailed health declaration form and provide documentary evidence in relation to the list detailed below. Where necessary health verifications will also require signature by a GP/Practice Nurse/Occupational Health Department. Please contact FAL if you have any questions or concerns.
Your identity
- National Insurance Number (P60 NI Card or wage slip)
- 2 passport size photographs
- 1 form of picture identity showing your name, current address (driving licence or passport)
- utility bill (not more than 3 months old)
- Criminal Records Bureau Declaration
Your Qualifications and Indemnity
- Photocopy of professional qualifications
- Photocopies of Certificates of any specialist training
- NMC pin number and expiry date
- ODP/HPC pin number and expiry date
- Most recent CPR Certificate
- Proof of Indemnity Insurance (photocopy or contact details of insurers and your reference)
Your health – verification of your results for:
- Heptatis B + blood test (including Titre levels)
- Rubella
- Varicella
- Tuberculosis
- Tetnus
- Diptheria
All candidates must complete an application form and health declaration and these must be posted direct to FAL. These can be downloaded or hard copies can be sent on request.
